Here's the unofficial link to the http://pavao.org/missions/Debriefing-Scramble.pdf. I'm hoping that it will reside somewhere more official in the near future, but if your hand-written log sheet rests as a blot upon your otherwise pristine character paperwork, here's a remedy for your predicament.
