So I'm curious - if a character has a limb blown off due to a critical success by an opponent, massive explosion etc. what options do they have to restore them? Can a spell just go poof you've got both legs back, do they need to be vat grown, do they need to be replaced...?

Custom spell I designed for one of my characters.
Just hack off some other guy's arm, get a needle and thread and some anti-rejection drugs and sew it on. Do it yourself in a motel bathroom with a magnifying glass, a button replacement kit, and no anesthetics to be even more badass.
You could potentially use Shapechange.

arguably maybe the drug that grants you regeneration power too . .
but both options are probably not good for the cybered folks . .
but for those replacing those fleshy bits with metals is most likely better anyway *g*
My favorite was always the "second hand limb" option from the SR1-3 rulebooks. Don't know if they kept it for SR4. But it's very Neuromancer, getting someone's used semi-diseased limb.
and people were complaining about how the healing rules of SR were unrealistic *snickers*
