Anyone have a good one? The couple that I found by looking for Fillable are dead links.

Well, mine isn't very good yet. I'm still working on it and many features are still missing. But perhaps you find it useful. Just look at my sig.

Mines M$ Word not PDF but your free to use it if you want just follow the link in the sig.

I made Isomodred's amazing 20 page dossier into a fillable PDF (yes, every field, blank, and checkbox). If you want, I'll ask his permission to post it for downloading somewhere.
