The tone of the game being run, or the personal code of ethics that particular player is adhering to (or NOT adhering to, as the case may be). GMs can try to set a particular tone for a campain, but, as many of the Dumpshock posters know, PCs are born to ruin the GM's best-laid plans. The way things would probably end up working, the GM who wants to keep the body count low would have a bunch of kill-crazed sammies and the GM who thirsts for the blood of the innocent will end up with a gaggle of hippie dove shamans.
